I have the factory "cigarette lighter" outlet on mine. I use a low profile USB insert in it that is snug. Sounds like you may have issues with one or both pieces. I'd try switching to a lower profile insert. If that is still loose you may have to spend more $ to get a better quality outlet

I've seen similar items on Amazon, I would have gone with something like that but previous owner spent the outrageous $ for the factory one
